  Task completion: measurement data is saved

  On completing a manual task, the values you send (**outcome, rating, time spent, note**) are now **saved and validated**. Before, on one of the completion channels, they were accepted then discarded — skewing metrics and ROI. Now required outcome/effort/feedback block completion until provided, and an outcome outside the allowed choices is rejected.

• Improvement

  Consistent required-field validation

  If a flow requires mandatory fields, the check now applies to **all ways of creating an item** (trigger, API/MCP, UI) — not just the trigger. No more items created without the required data.

• Design

  UI fixes

  The floating **"AI" button** no longer covers row controls in tables (e.g. the Actions column). `robots.txt` and `manifest.json` now return the correct content.
